Treatment of Children Trichobezoar a Retrospective Study of 11 Cases

Abstract

Purpose The goal of this study to summarize the clinical features, treatment and prognosis of children trichobezoar, and to guide the clinical diagnosis and treatment.

Methods The clinical manifestations, auxiliary examination results, diagnosis and treatment process and family relationship of 11 cases of children with trichobezoar in our hospital were analyzed retrospectively.

Results 11 cases were female, 4 cases were divorced single parent family, and 2 case was left behind child. Six patients were admitted to hospital with sudden exacerbation of chronic abdominal pain, and four of them had recurrent vomiting. Five patients were admitted to hospital with acute abdominal pain, and 3 of them had vomiting symptoms; The weight of 1 case was lower than −2 SD, 4 cases were between −2 SD ~ −1 SD. 6 cases had palpable left upper abdominal mass with mild tenderness, 1 case only had left upper abdominal tenderness, 4 cases had no positive abdominal signs; The results of color Doppler ultrasonography in 8 children indicated the strong echo mass in the stomach, and 3 of them showed that the hyperechoic group extended to duodenum through pylorus. 7 cases had a tail end extending into the small intestine after removing the gastrolith during the operation. Four cases were found with jejunal perforation, 2 cases were treated with intestinal resection and anastomosis, and 2 cases were treated with intestinal repair. All the children were improved and discharged after operation.

Conclusion For single parent families or left behind children with a history of chronic abdominal pain, vomiting and trichophilia, gastric hair stones should be highly suspected. For large hair stones in the stomach, they should be removed surgically. During the operation, the small intestine should be explored routinely to prevent the missed diagnosis of small intestinal perforation. Psychological guidance should be given to the children and their parents to prevent recurrence.

ZUSAMMENFASSUNG

Hintergrund und Ziel  Ziel der Studie war es, die klinischen Merkmale, Therapie und Prognose von Trichobezoaren bei Kindern zusammenzufassend darzustellen und Anleitung für die klinische Diagnostik und Therapie zu gegeben.

Methodik Wir analysierten retrospektiv die klinischen Manifestationen, Ergebnisse zusätzlicher Untersuchungen, das diagnostische und therapeutische Vorgehen sowie die Familienbeziehungen bei 11 Kindern mit Trichobezoar, die in unserem Krankhaus behandelt wurden.

Ergebnisse Alle 11 Fälle waren weiblich, 4 Fälle stammten aus Familien von geschiedenen Alleinerziehenden und in 2 Fällen war das Kind zurückgelassen worden. Sechs Patienten wurden mit einer plötzlichen Exazerbation chronischer Bauchschmerzen ins Krankenhaus aufgenommen; vier von ihnen litten an rezidivierendem Erbrechen. Fünf Patienten wurden mit akuten Bauchschmerzen ins Krankenhaus aufgenommen, davon 3 mit Erbrechen. In einem Fall lag das Gewicht unter − 2 SD, in 4 Fällen zwischen − 2 SD ~ und 1 SD. In 6 Fällen fand sich ein tastbarer Tumor im linken Oberbauch mit geringer Druckempfindlichkeit, in 1 Fall nur eine Druckempfindlichkeit im linken Oberbauch, in 4 Fällen keine auffälligen Bauchbefunde. Die farbkodierten Dopplersonographie zeigte bei 8 Kindern eine stark echogene Masse im Magen, 3 davon mit durch den Pylorus ins Duodenum reichender hyperechogener Gruppe. In 7 Fällen zeigte sich intraoperativ bei Entfernung des Gastrolithen, dass das Schwanzende sich bis in den Dünndarm erstreckte. In vier Fällen fand sich eine Perforation im Jejunum, in 2 Fällen erfolgte eine Darmresektion mit Anastomose und in 2 Fällen eine Darmrekonstruktion. Alle Kinder wurden nach der Operation in gebessertem Zustand entlassen.

Schlussfolgerung Bei Familien mit einem alleinerziehenden Elternteil oder zurückgelassenen Kindern mit chronischen Bauchschmerzen, Erbrechen und Trichophagie in der Anamnese, sollte unbedingt an das Vorliegen von Haarsteinen im Magen gedacht werden. Große Magensteine sollten operativ entfernt werden. Intraoperativ sollte routinemäßig der Dünndarm untersucht werden, um zu verhindern, dass eine Dünndarmperforation übersehen wird. Die Kinder und ihre Eltern sollten psychologisch betreut werden, um ein erneutes Auftreten zu verhindern.
